HOME VISITS 

All home visits will be triaged by a clinician in the first instance and will usually be carried out the same day as the request. 

Home visits are reserved for housebound patients only and all other patients would be expected to attend an appointment at the surgery. 

If you are temporarily housebound it is important that you let us know as soon as possible.

We offer home visits for all patients recorded as housebound. Home visits should be requested before 12:30pm where possible by contacting the surgery on 01226 282758. The patient co-ordinator will ask the nature of your condition. You must provide a contact number for the clinician to contact you on before the visit. If you have a key safe it is useful for us to have that information recorded on your medical record in case of emergencies.

We are unable to give a time for home visits as they could be any time on the day of request. If your visit needs to be moved to another day we will contact you to inform you of this.

Home visits for nursing procedures such as bloods, BP, dressings etc will be carried out by the District Nursing team which we can refer directly to. 

​

We may make the decision to refer you to the "Barnsley Healthcare Federation Home Visiting Service" for your home visit if we do not have the capacity to visit that day. You will still be visited by a GP or ANP and they will send the details to us to update your medical record.
