You can register as a patient with the practice by completing a registration form which can be obtained from reception or by clicking the link below for a PDF copy.

 Reception staff may ask you to have a seat and wait if you attend during a busy period before they can dedicate the time needed to check your registration form.

It usually takes around 5 working days for your registration to be processed, to avoid any delays please ensure you complete the entire form to the best of your knowledge. Forms may be rejected if important information is missing.  It is your responsibility to source the information required. 

You will be asked to complete a questionnaire which will provide us with important information regarding your health.  Newly registered patients are also recommended to attend for a health check with the Health Care Assistant (Adults only).  The health check is a method of introducing you to the practice and our services.

All patients that take repeat medication will be required to have a telephone consultation with the GP. Please ensure you have a months supply of medication from your current GP before registering with us.
